All providers at Wellspring at Port Orange Counseling are independent contractors who maintain and secure their own client records. Wellspring at Port Orange Counseling does not maintain or have access to individual client records.

Requests that do not identify the treating therapist may result in delays.

Upon receipt of a properly completed request and after consultation with the client, the individual therapist will review the request and respond accordingly.
